Hello! Thank you for your interest in booking with us! This form is for custom designs only, if you're looking to book a flash design, please DM your artist on Instagram instead!


PLEASE READ: This is NOT a booking form! This is a custom REQUEST, if your design idea does not fit our style, we may not accept the request, but will always try to recommend another artist in the area that we think would excel at what you're looking for!

Please include:

Subject matter (ex: rat, clown, etc), Style (ex: illustrative, woodcut, ignorant, blackwork, etc.), Placement (ex: ribs, bicep, calf, etc.), Rough size estimate (ex: 2 in, palm-sized, etc.)

If you have reference photos, please send them to your artist via cathedralcollectivelex@gmail.com OR through Instagram DMs.
